Cortisol is a hormone produced by the adrenal glands. Proper Cortisol levels are crucial for helping the body maintain blood pressure, glucose levels, immune response, and stress management. Cortisol levels tend to fluctuate throughout the day. Urine testing can provide a more accurate measurement as it evaluates the total cortisol production for an entire day, unlike a blood test, which will only show the cortisol level at the time the blood was collected. Cortisol testing is usually ordered to aid in the diagnosis of conditions related to the adrenal and pituitary glands such as Addison's disease, Cushing's syndrome, adrenocortical insufficiency, or hypersecretion.

Before beginning collection for this test, the person must receive a specimen container from the lab. Begin by urinating at 8:00 am and discarding the specimen. Then, collect all urine for a 24 hour period. Keep the specimen container refrigerated. After 24 hours, return the container to the lab.
