A quantitative immunoglobulin M (IgM) blood test identifies and monitors certain infections. This test is also commonly known as the antibody M test. Immunoglobulins are proteins produced by immune cells as a response to bacteria, viruses, or other foreign organisms. Increased IgM is associated with Waldenström's macroglobulinemia, infectious mononucleosis, viral infections, nephrotic syndrome, and estrogen therapy. Decreased IgM is found in selective IgM deficiency, Bruton's disease, and acquired immune deficiency. Immunoglobulin M is typically the first immune response to a new infection. The presence and measurement of IgM antibodies will determine if an infection is present and whether it is acute or chronic.
